SUMMARY OF MIDWIFERY CARE

One of the m ost im portant indicators of the success of m aternal and child health services is
the reduction of the M aternal Mortality Rate (MM R) and the Infant Mortality Rate (IM R). It is im portant
for m idwives to assess the health status and perform ance of m aternal and child health efforts.
M aternal and child health can b e seen from the M aternal M ortality Rate (M M R) and Infant M ortality
Rate (IM R). Midwives provid e Com prehensive Care starting from the p eriod of p reg nancy, the labor
process, birth, the postpartum period, and the use of contraception, aim ing to provide quality services
to reduce m orbid ity and m ortality in m others and child ren.

The m idwifery care p rovided to M rs. E.S was M idwifery Care, starting from the p eriod of
Pregnancy , lab or, postp artum , neonate, and fam ily p lanning at the Sipahutar Prim ary Health Center in
20 25, and docum entation was carried out using the SOAP form at.

The p regnancy care given to M rs. E.S. G3P2A0 was carried out in 2 visits w ith the 10 T
standard of service, and there were no com plications. Labor care in stage I was m onitored using a
partograph, stage II was ad justed to the 60 steps of N orm al Birth Care, stage III was carried out with
active m anagem ent of the third stage, and stage IV was m onitored for 2 hours at Sip ahutar
Com m unity Health Center, and no com plications were found.

Postpartum care w as carried out concurrently with neonate visits, w here the Postp artum Visit
(KF) for M rs. E.S. was carried out 4 tim es, and the N eonate Visit (KN ) was carried out 3 tim es in
Lum ban Rihit, Sipahutar sub-district, and no com plications were found during the postp artum period
and for M rs. E.S.’ s bab y.

This reason m otivated the author to p rovide com prehensive midwifery services starting from
the period of pregnancy, the lab or p rocess, newborn care, postpartum period , and fam ily planning
program directed to M rs. E.S G3P3A0. This service was im plem ented at Sip ahutar Com m unity H ealth
Center, Sipahutar Sub-district, North Tap anuli Regency, in 202 5.
