Abstract
Background: Pregnancy is a physiological process that requires
continuous monitoring to prevent pathological conditions that may
endanger the health of both the mother and the fetus.High-quality
maternal healthcare services, particularly during the third trimester, are
essential for managing common pregnancy-related discomforts such as
low back pain caused by fetal growth, uterine enlargement, and changes in
the body's center of gravity. Continuity of Care (CoC) provides a
comprehensive approach to improving maternal and neonatal health
outcomes. This study aimed to provide comprehensive midwifery care for
Mrs. W throughout pregnancy, childbirth, the postpartum period, newborn
care, and family planning.
Methods: This study employed a case study design using a continuity of
midwifery care approach. Midwifery care was continuously documented
using the Subjective, Objective, Assessment, and Plan (SOAP) framework.
Results: Mrs. W (G3P2Ab0) received four antenatal care visits during the
third trimester of pregnancy. The primary complaint was low back pain,
which was successfully managed through pregnancy exercise. She
experienced a normal vaginal delivery with a first-degree perineal
laceration, which was repaired using an interrupted suturing technique. A
female infant was delivered spontaneously with a vigorous cry, weighing
2,700 g and measuring 50 cm in length, with an APGAR score of 9/10.
Comprehensive newborn care was provided without complications. During
the 42-day postpartum follow-up, the mother experienced sore nipples
during breastfeeding, which improved following breast care education and
management. By the third postpartum visit, the nipple lesion had healed
completely, and the postpartum period progressed normally. Following
family planning counselling, Mrs. W agreed to become a three-month
injectable contraceptive acceptor.
Conclusion: Continuity of Care successfully supported the provision of
comprehensive midwifery care from pregnancy through childbirth, the
postpartum period, newborn care, and family planning in accordance with
midwifery care standards. Continuous implementation of this approach
may improve maternal and neonatal health outcomes. Mothers should
also be encouraged to attend Posyandu regularly to ensure that their
infants receive routine immunizations and exclusive breastfeeding for the
first six months of life.
