Abstract
Background: Continuity of Care (CoC) in midwifery refers to continuous care provided
to women throughout pregnancy, childbirth, the postpartum period, newborn care, and
family planning. This model of care is an important strategy for reducing maternal and
neonatal mortality by ensuring ongoing assessment and monitoring to optimize maternal
and fetal well-being.
Objective: To provide continuous midwifery care for pregnant women, women in labor,
postpartum mothers, newborns, and family planning clients using the midwifery
management approach.
Methods: A Continuity of Care (CoC) case study was conducted using the SOAP
(Subjective, Objective, Assessment, and Plan) documentation framework.
Results: Continuous midwifery care was provided to Mrs. R, a 29-year-old gravida III
para II abortus 0 (GIIIPIIA0), who attended four antenatal care visits. During pregnancy,
her primary complaint was lower abdominal pain. The first through fourth stages of labor
progressed normally, although a first-degree perineal laceration occurred. The infant was
delivered spontaneously, cried immediately after birth, weighed 3,568 g, measured 50 cm
in length, achieved an APGAR score of 9/10, and was female. The postpartum period
was uneventful, lactation was successfully established, and Mrs. R received counseling
on contraceptive methods and agreed to use the three-month injectable contraceptive.
Conclusion: The implementation of Continuity of Care enhanced maternal knowledge
and contributed to improved maternal and child health outcomes
