Abstract

Background: During the third trimester of pregnancy, women commonly experience physiological discomforts, including fatigue. Continuity of Care (CoC) midwifery services provide comprehensive care throughout pregnancy, childbirth, the postpartum period, newborn care, and family planning to improve maternal and fetal health outcomes.
Objective: To provide comprehensive Continuity of Care midwifery services from pregnancy through childbirth, the postpartum period, newborn care, and family planning in accordance with midwifery care standards.
Methods: This study employed a case study design using a Continuity of Care approach. Midwifery care was documented using the Subjective, Objective, Assessment, and Plan (SOAP) format.
Results: The client, Mrs. T, a 23-year-old woman (G2P1A0), attended four antenatal care visits. At the first visit (35–36 weeks of gestation), she reported fatigue as her primary complaint. Labor progressed normally during the first stage, while the second stage was complicated by a second-degree perineal laceration. A healthy male infant was delivered spontaneously with immediate crying, a birth weight of 3,650 g, a birth length of 50 cm, and an Apgar score of 9/10. The postpartum period was uneventful, lactation was successfully established, and the client received family planning counseling. She chose the 3-month injectable contraceptive as her preferred contraceptive method.
Conclusion: Continuity of Care midwifery services provided from pregnancy through family planning were successfully implemented in accordance with established midwifery care standards and contributed to improved maternal and neonatal health. Mothers are encouraged to bring their infants to Posyandu (integrated community health posts) for routine immunization services
