Background: Pregnancy, labor, puerperium, and newborn are physiological
conditions that may threaten the lives of mothers and babies and can even cause
death. One of the efforts that midwives can make is by implementing a
comprehensive / sustainable midwifery care model (continuity of care). With the
existence of comprehensive midwifery care can optimize the detection of high
maternal neonatal risk.
Objective: Implementing continuity of care midwifery care on Mrs. R aged 28
years in accordance with midwifery care standards and midwifery management.
Methods: Continuity of care method and documentation with SOAP
management.
Results: Midwifery care during pregnancy found Mrs. R's complaints of back
pain, but these complaints can be handled properly. At the time of delivery
everything went well and the newborn was spontaneous, cried immediately,
female sex, 3500 grams of weight, 50 cm PB, APGAR score 9/10 with a healthy
and fit condition there was a grade II perineal rupture and suturing was done. The
postpartum period of Mrs. R went normally and no complications were found and
during the postpartum visit family planning counseling was carried out with the
final result of Mrs. R choosing a 3-month injectable contraceptive.
Conclusion: Midwifery care provided starting from pregnancy to becoming a
family planning acceptor in accordance with the standards of care and the
authority of midwives.
