Abstract

Background: Continuity of Care (CoC) is a comprehensive model of midwifery
care provided continuously from pregnancy through childbirth, the postpartum
period, newborn care, and family planning using the midwifery management
approach with SOAP documentation. Low back pain is a common complaint
during the third trimester of pregnancy, primarily resulting from increased
abdominal load, changes in spinal curvature, and ligament stretching in the
lumbosacral region.
Objective: To improve maternal and child health through comprehensive and
continuous monitoring and midwifery care.
Methods: This study employed a case study design using the Continuity of Care
approach with SOAP documentation.
Results: Continuous midwifery care was provided to Mrs. S, a 36-year-old
gravida 3 para 2 abortus 0 (G3P2A0), who attended four antenatal care visits.
During pregnancy, she experienced low back pain, which was successfully
managed through Continuity of Care. The first and second stages of labor
progressed normally. However, a complication occurred during the third stage of
labor in the form of retained placenta. The placenta was not delivered despite
uterine bleeding, and manual removal of the placenta was attempted. As the
placenta remained undelivered for more than 30 minutes, referral procedures were
initiated in accordance with standard clinical protocols. The mother received
appropriate management based on the standards of midwifery care. The newborn
was delivered spontaneously, cried immediately after birth, weighed 2,800 g,
measured 48 cm in length, achieved an APGAR score of 9/10, and was female.
The postpartum period was physiological, lactation was well established, and Mrs.
S received counseling on contraceptive methods.
Conclusion: Continuity of Care midwifery services for Mrs. S were successfully
implemented in accordance with the standards of midwifery practice and provided
comprehensive care from pregnancy through family planning, thereby supporting
optimal maternal and neonatal health.
