Background: Antenatal Care (ANC) is an essential component of maternal
healthcare aimed at reducing maternal and neonatal mortality. Regular ANC
visits enable early detection of pregnancy complications. Physical changes
during pregnancy, especially in the third trimester, may cause frequent urination,
back pain, sleep disturbances,perineal discomfort,leg cramps,and ankle edema.
Objective: To provide comprehensive Continuity of Care midwifery services
for Mrs. V during pregnancy, childbirth, postpartum, newborn care, and family
planning.
Method: This study used a case study approach with Continuity of Care
midwifery
management.and.SOAP,(Subjective,Objective,Assessment,
Planning)documentation.

Results: Mrs. V, a 30-year-old G2P1A0 in her third trimester, underwent regular
antenatal examinations with no significant problems identified. During
childbirth, she experienced a second-degree perineal rupture, which was repaired
with suturing. No complications were found during the postpartum or neonatal
period. After receiving family planning counseling, she decided to use the 3
month injectable contraceptive.
Conclusion: Continuity of Care midwifery services improved maternal
knowledge regarding pregnancy and contributed to better maternal health
