Abstract
Background: Midwifery services play a vital role in safeguarding maternal and
neonatal health from pregnancy through family planning. Regular antenatal
monitoring is essential to assess maternal and fetal well-being and to facilitate early
detection of potential complications. Inadequate preparation during pregnancy,
such as insufficient breast care, may lead to breastfeeding problems, including
delayed milk production or flat nipples. Comprehensive midwifery care throughout
pregnancy, childbirth, the postpartum period, neonatal care, and family planning is
therefore essential to improve the quality and safety of maternal and neonatal health
services.
Methods: This case study employed a midwifery management approach with
documentation based on the Subjective, Objective, Assessment, and Plan (SOAP)
framework.
Results: Mrs. J (G2P1A0) attended four antenatal care visits. During pregnancy,
she presented with poor areolar hygiene and a fundal height inconsistent with
gestational age; both conditions were successfully managed. She had a normal
vaginal delivery complicated by a spontaneous second-degree perineal laceration,
which was repaired by perineal suturing. Four postpartum visits revealed no
complications. A healthy male infant was born with a birth weight of 3,200 g, a
length of 49 cm, and an Apgar score of 9/10. The infant was exclusively breastfed
and gained 1,300 g within 28 days after birth. The mother planned to use a three
month injectable contraceptive.
Conclusion: Continuity of Care midwifery services provided from pregnancy
through family planning were implemented according to established midwifery care
standards and contributed positively to maternal and neonatal health. Mothers are
encouraged to bring their infants to community health posts (Posyandu) for routine
immunization and to continue exclusive breastfeeding
