Abstract
Background: According to the World Health Organization (WHO), the
prevalence of anemia among pregnant women ranges from 20% to 70%,
indicating that approximately six out of ten pregnant women worldwide
experience anemia at 32–34 weeks of gestation. Approximately 40% of maternal
deaths are associated with anemia during pregnancy. Mild anemia may reduce
oxygen delivery to body tissues, resulting in pain and muscle cramps. Warm water
immersion therapy may improve blood circulation and relieve pain. Continuity of
Care (CoC) in midwifery aims to enhance maternal and neonatal health services
by providing comprehensive and continuous care from pregnancy through
childbirth, the postpartum period, newborn care, and family planning. This study
aimed to provide comprehensive midwifery care for Mrs. N throughout the
continuum of maternal and neonatal care.
Methods: This case study employed a midwifery management approach with
continuous documentation using the Subjective, Objective, Assessment, and Plan
(SOAP) framework.
Results: Mrs. N (G2P1A0) attended four antenatal care visits. At the first visit,
conducted at 33–34 weeks of gestation, she complained of hand and leg cramps
associated with anemia, which were successfully managed during pregnancy. She
experienced a normal vaginal delivery with a spontaneous first-degree perineal
laceration. Four postpartum visits revealed no complications. The newborn
weighed 3,100 g and measured 48 cm in length and was exclusively breastfed.
The infant gained 400 g within the first 14 days of life. Following counselling, the
mother planned to use a three-month injectable contraceptive.
Conclusion: Continuity of Care had a positive impact on maternal and neonatal
health outcomes. The implementation of comprehensive and continuous
midwifery care should be optimized, accompanied by education on warm water
immersion therapy to help alleviate muscle cramps during pregnancy.
