Abstract
Background: During the third trimester of pregnancy, women commonly
experience physiological discomforts, including frequent urination and lower
abdominal or low back pain resulting from uterine enlargement and normal
physiological changes. If left unmanaged, these conditions may affect maternal
comfort and well-being. Continuity of Care (CoC) is a comprehensive midwifery
approach that provides continuous care throughout pregnancy, childbirth, the
postpartum period, newborn care, and family planning. This study aimed to provide
comprehensive midwifery care for Mrs. S from pregnancy through family planning
services.
Methods: This study employed a case study design using a midwifery management
approach. Midwifery care was continuously documented using the
Subjective,Objective, Assessment, and Plan (SOAP) framework.
Results: The findings demonstrated that during the third trimester, Mrs. S
experienced frequent urination and low back pain, which were successfully
managed through health education, personal hygiene counselling, and pregnancy
exercise. She underwent a normal vaginal delivery with favorable maternal and
neonatal outcomes. A male infant was delivered with a birth length of 46 cm, a birth
weight of 2,900 g, and an APGAR score of 10/10. The newborn received early
initiation of breastfeeding (EIBF) immediately after birth. The postpartum period
progressed normally, with physiological uterine involution. Following family
planning counselling, Mrs. S chose a three-month injectable contraceptive as her
contraceptive method.
Conclusion: Comprehensive Continuity of Care was successfully implemented
throughout pregnancy, childbirth, the postpartum period, newborn care, and family
planning. Continuous provision of comprehensive midwifery care is recommended
to improve the quality of maternal and newborn health services and to support
optimal maternal and neonatal health outcomes
