Abstract

Background: Continuity of Care (CoC) midwifery services are based on an
ongoing partnership between a woman and a midwife. Comprehensive midwifery
care should be provided from the preconception period, throughout pregnancy,
childbirth, the first six weeks of the postpartum period, newborn care, and family
planning to optimize maternal and neonatal health outcomes.
Objective: To implement comprehensive Continuity of Care midwifery services
for a woman during pregnancy, childbirth, the postpartum period, newborn care,
and family planning using SOAP documentation.
Methods: This case study applied Helen Varney's midwifery management
approach with documentation using the Subjective, Objective, Assessment, and
Plan (SOAP) format.
Results: Mrs. S, a 28-year-old primigravida (G1P0A0), with a last menstrual period
(LMP) of 25 March 2026, attended three antenatal care visits. During pregnancy,
she complained of lower-extremity edema and low back pain. She experienced a
normal vaginal delivery, giving birth spontaneously at 7:00 p.m. to a healthy female
infant with a strong cry. Early Initiation of Breastfeeding (EIBF) was successfully
performed for one hour. The postpartum period progressed normally over 42 days,
with normal mobilization, urination, and defecation. The newborn had an Apgar
score of 9/10, a birth weight of 2,700 g, and a birth length of 50 cm. The infant
received 1 mg of vitamin K injection, the hepatitis B birth-dose (HB-0) vaccine,
colostrum feeding, and experienced normal umbilical cord separation. Family
planning counseling was provided, and Mrs. S chose the 3-month injectable
contraceptive as her preferred contraceptive method.
Conclusion: Continuity of Care midwifery services improved maternal knowledge
regarding pregnancy and contributed to better maternal health outcomes.
