Abstract

Background: Pregnancy, childbirth, and the postpartum period are physiological
processes; however, they require close monitoring because complications may
threaten the lives of both the mother and the fetus. One of the key indicators of
maternal and neonatal health is the reduction of maternal and infant mortality rates.
To achieve this goal, Continuity of Care (CoC) is essential, with antenatal care
serving as a fundamental component of comprehensive maternal health services to
improve maternal and fetal health outcomes. This study aimed to provide
comprehensive midwifery care for Mrs. N throughout pregnancy, childbirth, the
postpartum period, newborn care, and family planning in accordance with the
standards of midwifery practice.
Methods: This study employed a case study design using a continuity of midwifery
care approach. Midwifery care was continuously documented using the Subjective,
Objective, Assessment, and Plan (SOAP) framework.
Results: Mrs. N (G3P2A0), aged 30 years, received comprehensive midwifery care
through four antenatal care visits. At the first visit, conducted at 35–36 weeks of
gestation, her primary complaint was frequent urination, which was managed
appropriately during pregnancy. The first stage of labor progressed normally, while
the second stage was completed without perineal laceration. A female infant was
delivered spontaneously with a vigorous cry, weighing 3,200 g and measuring 51
cm in length, with an APGAR score of 9/10. The postpartum period progressed
normally without complications. Following family planning counselling, Mrs. N
agreed to become an implant contraceptive acceptor.
Conclusion: Continuity of Care successfully supported the provision of
comprehensive midwifery care from pregnancy through childbirth, the postpartum
period, newborn care, and family planning in accordance with the standards of
midwifery practice. Continuous implementation of this approach may contribute to
improved maternal and neonatal health outcomes through the early detection and
prevention of complications
