Abstract

Background: Comprehensive midwifery care is a continuous service provided by
midwives from pregnancy, childbirth, the postpartum period, newborn care, until
the mother decides to use a family planning method. The Continuity of Care
(CoC) approach aims to prevent complications during pregnancy, childbirth, the
postpartum period, newborn care, and family planning through integrated
maternity services.
Objective: To provide continuous midwifery care in accordance with the
standards of midwifery practice for Mrs. S, a 25-year-old gravida 1 para 0 abortus
0 (G1P0A0), from the third trimester of pregnancy through childbirth, the
postpartum period, newborn care, and family planning at Y.H Independent
Midwifery Practice, Pematangsiantar City.
Methods: A Continuity of Care (CoC) case study was conducted using the SOAP
(Subjective, Objective, Assessment, and Plan) documentation approach.
Results: Continuous midwifery care was provided to Mrs. S, aged 25 years
(G1P0A0), through four antenatal care visits. During pregnancy, her primary
complaint was frequent urination. Spontaneous labor occurred at 39 weeks of
gestation. The first through fourth stages of labor progressed normally, although a
second-degree perineal laceration occurred. A male infant was delivered
spontaneously, cried immediately after birth, weighed 3,900 g, measured 51 cm in
length, and achieved an APGAR score of 9/10. The newborn received
comprehensive essential neonatal care, including early initiation of breastfeeding

(EIBF), ophthalmic prophylaxis, vitamin K injection, hepatitis B birth-dose (HB-
0) immunization, and umbilical cord separation on the sixth day of life. The

postpartum period was uneventful, and Mrs. S received contraceptive counseling
and agreed to use the three-month injectable contraceptive.
Conclusion: Continuity of Care midwifery services using SOAP documentation
were successfully implemented, providing comprehensive care from pregnancy
through family planning in accordance with the standards of midwifery practice.
