Abstract
Background: Comprehensive midwifery care delivered through the Continuity of Care (CoC) approach is essential for continuously monitoring maternal and neonatal health and facilitating the early detection of complications. During the third trimester of pregnancy, women commonly experience physiological discomforts, including low back pain, which require appropriate management to prevent interference with daily activities. Objective: To implement comprehensive Continuity of Care (CoC) midwifery services for Mrs. D throughout pregnancy, childbirth, the postpartum period, newborn care, and family planning using the Subjective, Objective, Assessment, and Plan (SOAP) documentation method at Ariyani Bahliran Independent Midwifery Practice, Simalungun Regency. Methods: This case study employed a comprehensive midwifery management approach with SOAP documentation. Care was provided to Mrs. D, a 23-year-old primigravida (G1P0A0), from 32–33 weeks of gestation between February and May 2026. Results: During pregnancy, Mrs. D experienced low back pain, which was successfully managed through warm compress therapy and education on proper posture. Triple elimination screening yielded non-reactive results, and the mother's nutritional status was within the normal range. She had a spontaneous physiological vaginal delivery at 38–39 weeks of gestation at Sarimatondang Community Health Center. The newborn was delivered in good condition with a normal birth weight. Throughout four postpartum visits, uterine involution, lochia progression, and perineal wound healing progressed normally without signs of infection. During family planning care, the mother received counseling regarding contraceptive options. Conclusion: The implementation of comprehensive Continuity of Care midwifery services effectively minimized pregnancy-related discomfort, prevented complications, and ensured that the reproductive process proceeded safely and physiologically for both the mother and the newborn
