Asuhan Kebidanan pada Ny. W Masa Hamil, Bersalin, Nifas, Bayi Baru Lahir, dan Keluarga Berencana di Praktek Mandiri Bidan I.S Pematangsiantar

Background: The prevalence of pregnant women with anemia according to
Riskesdas in 2018 has increased significantly from 2018. High-risk pregnancies
are found in pregnant women who are too old, too young, too numerous and too
close (4T).
Purpose : To provide midwifery care to Mrs. W aged 38 years with continuity of
care starting from pregnant women, childbirth, postpartum, newborns, and
family planning in accordance with the standards of midwifery care and
midwifery management.
Methods : Midwifery care is managed and documented with SOAP management.
Results : In Mrs. W's case, she had mild anemia and a high risk of pregnancy. The
management was carried out using a midwifery care management approach to
Mrs. W starting from the third trimester of pregnancy to family planning at the
Independent Practice of Midwife IS Pematangsiantar from 04 February to 06 April
2021. The care provided to Mrs. W was Ante Natal Care (ANC) visits. 3 times.
Childbirth care from the first to the fourth stage, Mrs. W experienced a grade I
rupture, but hecting was done with a broken knot technique. The puerperium was
performed 3 times and there were no problems and complications. Newborns
were given IMD, HB0 immunization, newborn care and 3 visits and no
complications were found in newborns. Through family planning counseling, Mrs.
W decided to use Implant.
Conclusion: In this case Mrs. W age 38 years with mild anemia, high risk of
pregnancy, and perineal rupture. There is no gap between theory and practice.
