Background: Comprehensive midwifery care is a continuous service provided by
midwives from pregnancy, childbirth, the postpartum period, newborn care, until
the mother decides to use a family planning method. This Continuity of Care
(CoC) approach aims to ensure integrated maternity services while preventing
complications during pregnancy, childbirth, the postpartum period, newborn care,
and family planning.
Objective: To provide continuous midwifery care based on the standards of
midwifery practice for Mrs. L, a 21-year-old gravida 2 para 1 abortus 0
(G2P1A0), from the third trimester of pregnancy through childbirth, the
postpartum period, newborn care, and family planning at W.P Independent
Midwifery Practice, Batu VI Laras II, Simalungun Regency.
Methods: A Continuity of Care (CoC) case study was conducted using the SOAP
(Subjective, Objective, Assessment, and Plan) documentation approach for
midwifery care.
Results: Continuous midwifery care was provided to Mrs. L, aged 21 years
(G2P1A0), from the third trimester of pregnancy through childbirth, the
postpartum period, newborn care, and family planning. Throughout pregnancy,
both maternal and fetal conditions remained normal. Labor progressed normally
without complications. The baby was delivered spontaneously, cried immediately
after birth, was active, and showed no congenital abnormalities. The postpartum
period was physiological, with normal uterine involution and successful lactation,
while the infant demonstrated appropriate growth and development. At the
completion of care, Mrs. L received a family planning service tailored to her
reproductive needs.
Conclusion: Continuity of Care midwifery services for Mrs. L were successfully
implemented in accordance with the standards of midwifery practice and were
completed without significant maternal or neonatal complications. Mothers are
encouraged to maintain a healthy lifestyle, meet their nutritional requirements,
attend routine maternal and child health services, provide exclusive breastfeeding,
and participate in family planning programs as recommended by healthcare
professionals.
