Abstract

Background: Efforts to improve midwifery care can be achieved through the
implementation of the Continuity of Care (CoC) model. Continuity of Care in
midwifery services refers to an ongoing relationship between a woman and a
midwife,providin comprehensive care from pregnancy, childbirth, postpartum,
newbore care, to family planning.
Objective: To provide comprehensive Continuity of Care midwifery services
during pregnancy, childbirth, postpartum, newborn care, and family planning.
Method: This study used a case study approach with Continuity of Care
midwifery management and SOAP documentation.
Results: Mrs. R received a series of antenatal care (ANC) examinations. Her
complaint of third-trimester back pain was physiological and was successfully
managed. She had a normal spontaneous vaginal delivery. No complaints were
identified during postpartum visits. The newborn was healthy, cried immediately
after birth, and had an APGAR score of 9/10. After family planning counseling,
Mrs. R chose the 3-month injectable contraceptive.
Conclusion: The implementation of Continuity of Care for Mrs. R had a positive
impact on improving maternal knowledge and optimizing maternal and child
health. It was also effective in the early detection and management of phys
iological conditions according to midwifery care standards.
