Abstract
Background: Antenatal care (ANC) is an essential strategy for monitoring the
health of both pregnant women and their fetuses. To ensure comprehensive
maternal and neonatal care, the Continuity of Care (CoC) model provides
integrated services throughout pregnancy, childbirth, the postpartum period,
newborn care, and family planning. The CoC philosophy emphasizes the natural
childbirth process by supporting women to give birth with minimal intervention
while addressing their physical, psychological, spiritual, and social well-being.
Objective: To provide comprehensive Continuity of Care (CoC) midwifery
services for a woman during pregnancy, childbirth, the postpartum period,
newborn care, and family planning.
Methods: This case study applied the Continuity of Care approach with SOAP
documentation. Care was provided to Mrs. W, a 25-year-old woman (G2P1A0),
from 32–33 weeks of gestation between February and May 2025.
Results: Mrs. W completed four antenatal care visits. During pregnancy, she
experienced Bell's palsy and low back pain, both of which were successfully
managed through education and close monitoring. She had a normal vaginal
delivery with a second-degree perineal laceration that was treated using layered
suturing. Breast engorgement developed on the sixth postpartum day but resolved
following appropriate management. The newborn had a birth weight of 3,000 g
and a length of 49 cm and received a 1-mg vitamin K injection and the hepatitis B
birth-dose (HB-0) vaccine. At 23 days of age, the infant had gained 1.2 kg and
subsequently received Bacillus Calmette–Guérin (BCG) and the first dose of oral
polio vaccine at one month of age. Following family planning counseling, the
mother chose an implant as her contraceptive method.
Conclusion: Comprehensive Continuity of Care midwifery services were
successfully implemented throughout pregnancy, childbirth, the postpartum
period, newborn care, and family planning. All identified maternal complaints and
clinical problems were appropriately managed, resulting in favorable maternal and
neonatal health outcomes. Strengthening continuous midwifery care is essential to
support the early detection of complications and improve maternal and child
health.
