Abstract
Background: Pregnancy, childbirth, and the postpartum period are
physiological processes; however, they require close monitoring because
complications may threaten the lives of both the mother and fetus.
Reducing maternal and neonatal mortality remains a key indicator of
health status. Continuous healthcare services, including routine antenatal
care, are essential to promote maternal and fetal well-being and facilitate
early detection of complications.
Objective: To provide comprehensive Continuity of Care midwifery
services from pregnancy through childbirth, the postpartum period,
neonatal care, and family planning in accordance with established
midwifery care standards.
Methods: This case study applied the Continuity of Care model using the
Subjective, Objective, Assessment, and Plan (SOAP) documentation
approach.
Results: Mrs. N, a 29-year-old woman (G4P2A1), attended three antenatal
care visits. At the initial visit (35–36 weeks of gestation), she complained
of fatigue. Labor progressed normally, although a second-degree perineal
laceration occurred during the second stage of labor. A healthy male infant
was delivered spontaneously with an Apgar score of 9/10, a birth weight of
3,200 g, and a body length of 47 cm. The postpartum period was
uneventful, breastfeeding was successfully established, and the mother
received counseling on contraceptive options before choosing condoms
as her family planning method.
Conclusion: Continuity of Care midwifery services provided from
pregnancy through family planning were successfully implemented
according to midwifery care standards and contributed positively to
maternal and neonatal health. Mothers should be encouraged to bring
their infants to community health posts (Posyandu) promptly to receive
routine immunizations
