Abstract

Background: Comprehensive midwifery care is a continuous model of maternity
care provided from pregnancy through childbirth, the postpartum period, newborn
care, and family planning services. This approach aims to improve the quality and
safety of maternal and neonatal healthcare through integrated Continuity of Care
(CoC).
Methods: A Continuity of Care (CoC) case study was conducted using the SOAP
(Subjective, Objective, Assessment, and Plan) documentation framework.
Results: Continuous midwifery care was provided to Mrs. J, a 36-year-old
gravida 2 para 1 abortus 0 (G2P1A0), through four antenatal care visits. During
pregnancy, her primary complaint was frequent urination. Midwifery
interventions included education on appropriate fluid intake, avoiding delayed
urination, maintaining genital hygiene, and engaging in light physical activity,
such as pregnancy exercise, to reduce pressure on the bladder. These interventions
effectively alleviated the complaint and improved maternal comfort throughout
pregnancy. Spontaneous labor occurred at 39–40 weeks of gestation. The first
through fourth stages of labor progressed normally, with a first-degree perineal
laceration. A male infant was delivered spontaneously, cried immediately after
birth, weighed 3,200 g, measured 50 cm in length, and achieved an APGAR score
of 9/10. The newborn received comprehensive essential neonatal care, including
early initiation of breastfeeding (EIBF), ophthalmic prophylaxis, vitamin K
injection, hepatitis B birth-dose (HB-0) immunization, and umbilical cord
separation on the fourth day of life. The postpartum period was physiological, and
Mrs. J received contraceptive counseling and chose the three-month injectable
contraceptive as her family planning method.
Conclusion: Continuity of Care midwifery services using SOAP documentation
were successfully implemented, providing comprehensive and standardized care
from pregnancy through family planning while supporting favorable maternal and
neonatal outcomes
