Background: Continuity of care (COC) is essential throughout pregnancy,
childbirth, the postpartum period, newborn care, and family planning to improve
the quality of care and prevent obstetric complications. Objective: To implement
continuous midwifery care (Continuity of Care) for Mrs. M using SOAP
documentation at the T.H. Independent Midwife Practice (PMB) in
Pematangsiantar City. Methods: This case study employed a comprehensive
midwifery management approach for Ms. M, a 36-year-old woman (G3P1A1),
beginning at 36 weeks of gestation and continuing through labor, the postpartum
period, the newborn care phase, and family planning. Results: During the third
trimester, she reported physiological complaints of lower back pain, which were
successfully managed through education on improving body posture. The delivery
process proceeded spontaneously and normally at 38–39 weeks of gestation, aided
by the birth ball method to accelerate fetal head descent. The baby was born
healthy, and Early Breastfeeding Initiation (EBI) was performed immediately for
1 hour; the infant weighed 3,600 grams and measured 48 cm in length. The
postpartum period proceeded normally without obstetric complications, uterine
involution was good, and the mother received family planning counseling.
Conclusion: Continuity of care for Ms. M was effectively provided from
pregnancy through family planning, with no significant gaps between theory and
practice.
