ASUHAN KEBIDANAN PADA NY. E MASA HAMIL, BERSALIN, NIFAS, BAYI BARU LAHIR DAN KELUARGA BERENCANA DI PRAKTIK MANDIRI BIDAN TIANGGUR KOTA PEMATANGSIANTAR

Background and Objective: Continuity of Care in midwifery is
comprehensive care provided from the third trimester of pregnancy through
childbirth, postpartum, newborn care, and family planning. Common third
trimester discomforts include constipation, edema, insomnia, and back pain.
Pregnancy at an advanced maternal age (≥35 years) carries higher risks. This
study aimed to provide continuous midwifery care for Mrs. E.
Method: A case study using Continuity of Care with SOAP documentation.
Results: Mrs. E, a 41-year-old G2P1A0, received three antenatal visits.
Back pain and frequent urination were managed successfully. Delivery was
uncomplicated except for a first-degree perineal tear repaired with
interrupted sutures. No postpartum or neonatal complications were found.
After counseling, Mrs. E chose an implant contraceptive.
Conclusion: Continuity of Care had a positive impact on maternal and
newborn health. Mothers are encouraged to apply the recommendations
given to maintain maternal and infant.health
